I am committed to protecting your privacy in accordance with the Data Protection Act 1998 and GDPR. Under these Acts, I have a legal duty to protect any information I collect from you. I take appropriate security measures to safeguard your data and apply security standards & controls to prevent any unauthorised access to it.
Information I may collect from you:
I may collect and process the following data about you:
Information you give me. You may give me information by corresponding with me by phone, email or otherwise. The information you give may include your name, address, email address and phone number, personal description, medical history, training needs. Please let me know if you would prefer that I did not store such data in your case.
Information I collect about you
With regard to each of your visits to the website, it may automatically collect, as most websites do, the following information:
1. technical information, including the Internet protocol (IP) address used to connect your computer to the Internet, browser type and version, time zone setting, browser plug-in types and versions, operating system and platform;
2. information about your visit, including the full Uniform Resource Locators (URL) clickstream to, through and from our site (including date and time); pages you viewed or searches you carried out; page response times, download errors, length of visits to certain pages, page interaction information (such as scrolling, clicks, and mouse-overs), and methods used to browse away from the page.
Uses made of the information
I use information held about you in the following ways:
1. to carry out my obligations arising from any contracts entered into between us relating to training, counselling & supervision consultations and to provide you with the information and services that you request from me;
2. to notify you about changes to my services;
Information I collect about you in a counselling session and how it is used
Upon starting counselling, basic personal information will be collected for contact and identification reasons. During counselling meetings, an assessment of your psychological health will be completed, and notes will be taken after sessions. These will include personal and sensitive details about your life. The assessment and notes are used solely for the delivery of a therapy service to you.
Your therapy and personal information is kept securely. Information but not names will be shared with my supervisor who is also accredited and who regularly reviews my practice. Confidentiality will be broken if I have concerns that you or anyone else is at risk. If this occurs it will be discussed in the session where possible, and actions or recommendations will be discussed and documented in your notes.
Disclosure of your information
Information that is disclosed to other parties is only upon commencement of a therapy contract and on the conditions as specified in the Counselling Agreement.
Information I collect about you in a supervision session
During the supervision sessions, themes, specific focus and process notes will be taken. The notes
are used solely for the delivery of a supervision service to you.
Information I collect about you when you undertake training
will be kept for up to three years after the completion of the course, so that I can notify you of any additional relevant training opportunities. It will be then deleted or destroyed as applicable, unless you request to remain on the database for longer
The information you supply on your application form
The content of your portfolio will be kept for up to one calendar year after the submission date so that it is available for inspection by the OCN external moderator, after which time, it will be destroyed.
Where I store your personal data
Once I have received your information, I will use strict procedures to protect your data on my systems.
Your personal data will not be used for marketing purposes, nor shared with any third parties. I will not share any of your confidential medical information with any third parties unless I am instructed or permitted to do so by you or are required to do so by law.
You have rights relating to the information I hold to verify the accuracy or to ask for them to be supplemented, deleted, updated or corrected. You have the right to request a copy of the information that I hold about you. If you would like a copy of some or all of your personal information, please email or write to me via the contact details stated on this website. Information will be provided to you within 30 days.
I want to make sure that your information is accurate and up to date. You may ask me to correct or remove information you think is inaccurate. You have a right to request the transfer of your data to another individual or company.
Sharing of data
I store client contact details on my personal computer, which requires a password before data can be accessed. Please let me know if you would prefer that I did not keep such data in your case. Your contact details are only seen by me.
I use conventional domestic email, which like all such services is not specially encrypted. If you are concerned about the breaches of privacy that might potentially occur in any such system, then please do not mail sensitive information, or choose an alternative means of communication. On rare occasions where your information needs to be shared with 3rd parties, I will explicitly ask your consent before doing so, and the data will be sent to 3rd parties securely.
Security of your data
Information will be kept securely and confidentially in line with the data retention policy as stated above. All paper notes are kept in a secure, filing cabinet and stored within a secure building. All digital information is stored on a single domestic computer, which is password protected and stored within a secure building. My mobile phone is encrypted.
Lawful basis for processing your information
The lawful basis for my holding and using your information is in relation to the delivery of a contract to you as a health care professional. As a senior accredited member BACP, I operate under a strict code of confidentiality.
